What you get: 1-hour visit with your physical therapist: Evaluation, development of plan, home exercise program in writing, or pictures.

What you get:  3, 1-hour sessions with your physical therapist.

What you get:  6, 1-hour sessions with your physical therapist.

Wellness Maintenance package: $1450 paid in advance, or $130/month by Credit Card Withdrawn on the 5th of each month for 12 months.  (please see full description)
